Description: There is a gap in our understanding of how teams are working across settings to support care continuity and coordination. This is an important topic since having multiple interprofessional teams in the healthcare system is a new and emerging phenomenon. This scoping review identified the scope of literature reporting on the use of multiple interprofessional teams to support care transitions from hospital to home for older adults, and synthesized the strategies being used by these teams to support care transition coordination.
